The Medieval Half Marathon begins in the heart of Kilkenny City at the Tholsel on High Street, offering a grand departure through the city's ancient core. The course transitions from narrow medieval lanes to the scenic, winding country roads of County Kilkenny, passing by the majestic Kilkenny Castle and crossing the River Nore. Runners face a terrain defined by gentle rolling hills and long, straight sections that offer a rewarding challenge without being overly taxing. The route brings participants back toward the city's vibrant center, passing St. Canice's Cathedral before a triumphant finish at the Castle Park or along The Parade.

World Athletics record-eligibility rules cap net elevation loss at 1m per km of race distance, and require the start and finish to be no more than 50% of the race distance apart — both meant to stop a course gaining an unfair edge from downhill grade or a point-to-point tailwind.
